Deflation, Bilateral Reaugmentation with Larger Saline Implants, Peri-areolar Mastopexy

46 y/o woman with saline implants done elsewhere, unknown size and manufacturer, wished slightly larger size and better appearance with lift. She presented with left-sided saline deflation. Treatment consisted of bilateral capsulotomy (enlargement of the pocket); re-augmentation with Allergan 360 saline MP filled to 390, and periareolar mastopexy. Photos are 3.5 months post-op.

Dr. Robert Lowen/ October 3, 2021 I would like to begin by saying that I am no stranger to plastic surgery. However, I am now in my early seventies and have had three breast augmentations and/or adjustments over the years. I ended up with breasts that were way too large, not symmetrical and with a dented, tethered nipple, caused by internal scar tissue from the previous periareolar incision. I had 575 cc silicone on the tethered side and 450 silicone on the other. Three different doctors long story. . . My last surgery was in 2009. I found “RealSelf” website and researched, studied, obsessed and read countless testimonials and reviews, looked at before and after photos of women of all ages, hoping for some kind of match and possible guidance. I did this for almost a year and I kept coming back to one doctor. . . Dr. Robert M. Lowen in Mountain View, California. What I did know for sure, after all that time, was that I wanted a serious, skilled surgeon and not a song and dance man. The industry is filled with them. They hide in plain sight so you have to do your research. Also, I was now older, or seasoned as they say, and I wanted this to be my last big general anesthesia surgery. Finally, I knew my case was complicated. I went ahead and made an appointment for a consultation with Dr. Lowen. Before my appointment I read his Bio or “Meet Dr. Lowen” from his actual website so I could feel like I kind of knew him a little beforehand. Let me say that his history or story, credentials and accomplishments are really impressive. He is also listed as a “Diplomate, American Board of Plastic Surgery” which the Board defines as a significant Professional Achievement and Honor. Also, on his website I read that he specializes in breast procedures among other surgeries. He studies the latest technology and techniques by taking specialized teaching courses throughout the year. He has traveled around the world (even India) practicing and perfecting his skills. He studied and learned new techniques and instrumentation for “Rapid Recovery Breast Augmentation” and Close to Bloodless Procedures.” Yes, it is a thing, as you will find many reviews on his website and RealSelf talking in amazement about his “drainless tummy tucks” and “drainless breast surgeries”. I knew that Dr. Robert Lowen was the doctor for me and I hadn’t even met him yet. When we did meet, I immediately felt at ease. He is serious and focused on what needs to be done, but at the same time had a very relaxed and comfortable energy. He has over 30 years of experience. I already knew that Dr. Lowen was interested in education, research and innovation, but he also seems humble and caring. I feel he really wants his patients to be happy with their surgeries. He runs a tight ship with Covid protocol as well. He definitely listens to what you want but is truthful about what can be done. Even with that, he still strives for perfection. I am so happy with my results and grateful to him. He removed the 575 cc silicone on the right and replaced it with a 315 cc saline “Ideal Implant” and repaired my tethered nipple from the inside. Amazing! On the left he replaced the 450 cc silicone implant with a 285 cc saline “Ideal Implant” as well. He stitched down and repaired my breast pockets to accommodate the new smaller implants. Only 30 cc’s difference now instead of the 125 cc from before. My breasts look wonderful. No major cutting or breast lift, and at my age that in itself is totally amazing. Also, I had a quick recovery. No drains and only one pain pill!! As a side note, Dr. Lowen told me that his grandfather and his grandfather’s five brothers were all master tailors. He said his grandfather was a real gentleman and a nice man. I thought that was really fascinating and that genetics must certainly play a role in his family for sure, judging from his incisions and stitching skills from my surgery. It is almost a month and my inframammary incisions, slightly over an inch, are clean and almost flat. Also, I want to mention Ayana. She was absolutely wonderful and kind and helpful. She is the OR nurse who works with Dr. Lowen. She helped me navigate with the paperwork and emails in preparation for surgery as well. She is the one who corrected me when I said I was old, and she told me to say I was seasoned. A very nice person. Dr. Lowen told me he loves plastic surgery and he especially loves challenging surgeries. I am so happy with my smaller, symmetrical, younger looking breasts and so happy I found Dr. Robert Lowen.
