This 54- year-old woman, 5’5″, 183 lbs., BMI 31.5, had a drainless tummy tuck and power-assisted SAFE liposuction of the abdomen, waist, flanks and bra rolls. The umbilical donor site was sutured as a small vertical closure in the lower abdomen, as she had insufficient upper abdominal laxity to pull the umbilical donor site down far enough to remove it. Her scar is healing as a fine, flat line. She also used embrace scar therapy. No drains were used because the flap was re-sutured to the abdominal wall with progressive tension sutures, which relieve tension at the incision line, leading to more favorable, tension free conditions for the skin scar as well as eliminating the need for drains. Exparel was injected under the abdominal fascia and the liposuctioned areas at the time of surgery, giving sufficient relief from post-operative pain that a pain pump was not needed. Instead of having four catheters for pain pump and drains, after surgery she had none and was immediately able to get up and move around unencumbered.
